Purpose

This NDIS Participant Management & Onboarding policy
(“policy”) will help you understand how Serenity Counselling (“us”, “we”, “our”) manages the  commencement of services with NDIS participants. It also explains how we gather and use participants plan information to assist with their therapeutic goals and reporting.

Policy Standard

Serenity Counselling is committed to providing  accessible, high-quality, person-centered therapeutic support to National Disability Insurance Scheme (NDIS) participants. To ensure clarity, transparency, and clinical alignment, a formal intake process must be completed prior to the commencement of therapeutic services.

Scope & Provider Status

Serenity Counselling delivers counselling services under the capacity-building support categories.

We welcome participants whose NDIS funds are Self-Managed or Plan-Managed.

Please note that Serenity Counselling is not a registered NDIS provider; therefore, we cannot accommodate participants whose funds are directly agency-managed by the NDIA.

Mandatory Intake Requirements

Before booking an initial therapeutic session, the following documentation must be completed and processed:

·         NDIS Referral Form: A completed referral form is required to help us understand the participant’s goals, funding structure, and support needs.

o    Plan-Managed Participants: The form may be completed and submitted by the participant’s Support Coordinator or Local Area Coordinator (LAC).

o    Self-Managed Participants: The form may be filled out directly by the participant, or a designated family member/nominee.

·         Service Agreement: In alignment with NDIS best practices, a formal Service Agreement must be established and signed by both Serenity Counselling and the participant (or their authorized representative/plan manager) before service delivery begins. This agreement will explicitly detail:

o    The schedules and frequency of supports including any expected reporting requirements.

o    The agreed hourly rates (in accordance with permitted pricing limitations/NDIS price guide limits for counselling).

o    Billing and payment terms.

o    The cancellation policy.

o    Processes for reviewing or ending the agreement.

Service Delivery & Goal Alignment

All therapeutic interventions provided by Serenity Counselling will be directly mapped to the individual goals outlined in the participant’s current NDIS plan. Progress documentation and any requested outcomes reporting will be maintained to assist participants with their scheduled NDIS plan reviews.
